For providers

Clinics are invited to join the Advocacy Clinic Consortium. As part of the Advocacy Clinic Consortium, your clinic will contribute aggregated, de-identified real-world data on the efficacy of ketamine for CRPS patients. No data will ever be traceable back to a clinic or an individual patient and will never be shared between clinics. This is not a research study, but a real-world evidence consortium, in which anonymized and aggregated charts will be retrospectively analyzed to examine outcomes in naturalistic clinical care settings. While we will help recruit pain patients to your clinic, we do not mandate or require any protocols.

Results will be packaged and sent to the Center for Medicare and Medicaid Services (CMS) as part of an overarching application for reimbursement coverage that is already in progress.

If you'd like to participate, please fill out the below contact form. A member of the Taskforce will reach out and onboard you to Osmind EHR for documentation purposes.

We greatly appreciate your willingness to participate and contribute to this cause - we hope to serve you as best we can.

Thanks for submitting!

You can also email ketaminetaskforce@gmail.com

For patients

We would love to include you in our consortium! We are able to work with providers and patients who are currently on Osmind. We encourage you to ask your provider if they are part fo the Advocacy Clinic Consortium contributing their aggregated, deidentified information to research. This will allow you to contribute your information to the cause.

If they are not, please fill out the below information with and we will reach out to your clinic. Please also encourage your provider to sign up on this website. They can always email ketaminetaskforce@gmail.com for more information as well.

As a reminder, we will be aggregating, de-identifying real-world data on the efficacy of ketamine for CRPS patients like you. No data will ever be traceable back to you as an individual or shared with others.

If you are part of an Advocacy Clinic, your provider may ask you to be recording certain information such as filling out pain scores and Katz scales in Osmind. We recommend complying with this so we can gather as much information as possible to share with Medicare!

We greatly appreciate your willingness to participate and contribute to this cause - we hope to serve you as best we can.
